Medical Diet Request From

This Medical Diet Request Form that is required by the facility for anyone with a medical diet request. This is NOT for meal or dietary preferences, but rather food allergies where if consumed, you would need medical attention. Please be specific on your form.

If you have medical diet request, please print and return the completed form to the Shop in person OR you can email it to teamAQR@amysquiltroom.com

Please NOTE, even if you recently attended an event with us, we need you to do this for each event to be sure we have the most current information. If we **DO NOT** receive this completed form returned by the deadline, we will assume you do not have any Medical Diet Requests.
